Long Beach, Calif.-based Dental Health Services suffered an email data breach affecting 4,562 members.
On Feb. 7, the company mistakenly emailed monthly invoices containing personal information to some of its employer group customers. The emails were password-encrypted, but the recipients were also sent an email containing the password, according to a July 10 news release.
Member information contained in the invoices included members' names, dates of birth, member identification numbers, eligibility dates, plan types and premium amounts due.
All of the email recipients confirmed destruction of the incorrect invoices and the company has no evidence that any information has been misused.
Dental Health Services is a dental benefits company operating in California, Oregon and Washington, the release said.
