Alexandria, Va.-based Mount Vernon Dental Smiles recently experienced a data security incident involving patient information.
The practice said that on Aug. 23 it inadvertently attached a spreadsheet containing patient data on a patient communication email. The practice then emailed patients to inform them of the error and request that the previous email be deleted.
The practice said it has no evidence that the patient information was misused.
The potentially affected information may include contact information, identifying information, dates of service, health insurance information and provider names, according to a Jan. 19 news release.
The practice said it notified affected individuals of the incident by mail and has implemented measures to minimize the risk of similar incidents happening again.
